Built for healthcare. Focused on relationships. Operating nationwide.
Primary care doctors, ER social workers, discharge planners, and case managers refer patients to therapy and psychiatry every day. They send them to the few practices they already know. We get you introduced, warmed up, and onto that list.
Your calendar has room. The right referrals just aren’t reaching you.
You are licensed. You are credentialed with the plans. Your calendar has room. 137 million Americans live in a federally designated mental health shortage area. You still have open slots this week.
The gap sits between you and the people who route patients. PCPs who screen for depression with no therapist down the hall. ER social workers who must hand off before discharge. Case managers who can only send in network.
They refer to names they know. If they don’t know yours, or don’t know you take their patient’s plan, the referral goes somewhere else.
So practices lean on directory listings, ZocDoc, and platform panels. The patient comes through. The relationship never does. Caseloads rise and fall, and hiring waits.
We handle everything before the introduction and everything after it. Your job is to show up, make a good impression, and deliver great service.
We identify the hospital discharge planners, SNF social workers, and elder care advisors in your exact radius who actively place seniors matching your care levels.
Our team introduces your community, licensing, and payer acceptance ahead of time so you walk into every conversation as a known, trusted operator.
When a referral partner is ready to connect, we place the meeting directly on your calendar. You build the relationship and fill open slots with no per-referral fees.
What we build for you in the first three weeks:
Clear answers to these four questions make you easy to refer to.
A case manager cannot route a patient to a provider who cannot bill their plan. Your panels decide which partners can refer to you at all.
Health plans are scored on whether patients leaving a psychiatric hospital stay see a mental health provider within 7 days. Discharge teams remember who can take the appointment.
TMS, MAT, eating disorders, couples work, SAP evaluations. Referral sources send complex cases to the provider they know handles them, and most don’t know yours yet.
A referring clinician wants to know the patient showed up and is improving. The practice that reports back is the one they call again.

“Honestly, I had my reservations going into this process with PractiScale, but they have truly proved me wrong by continuing to deliver exactly what they promised us during that initial call.”

Steady referrals let you add a clinician on a plan, not on a hunch.
Partners who know your openings fill them before they go unused.
Your pipeline stops rising and falling with a listing or a panel.
A warm handoff from a clinician they trust shows up more often than a cold inquiry.
We never take a percentage of the patient’s your partners send.
The CRM, the pipeline, and the relationships are built in your practice’s name.
“Picture a Monday morning. An ER social worker has a patient going home today who needs a therapist this week. She calls you first, because she knows you take the plan and you have openings.”
That is what a working referral network looks like. That is what we help you build.
Every referral partner we connect you with is one you have approved. You meet them, you decide if they are the right fit for your specialties and the plans you accept, and you say yes before we move forward.
Referral relationships are not always perfect from day one. If a partner we connect you with slows down, stops sending patients, or turns out not to be the right fit we go back to work. We find a replacement, warm them up, and make the introduction again. No extra cost. No long-term contract holding you in place.
We stay involved until you have referral partners who are actually sending patients consistently.
If a partner stops performing, we find a replacement. No extra cost.
You stay because it is working, not because you are locked in.
You meet them. You decide. You approve.

Yes. Therapy, psychiatry and medication management, TMS and ketamine, MAT and addiction treatment, and telehealth practices. Your partner map follows your services and your license states.
No. Most NEMT companies we work with keep their broker contracts while building direct referral relationships alongside them. The goal is reducing your dependency on brokers so a slow broker week does not become a slow business week.
It matters a lot. We target partners whose patients carry the plans you accept. Private pay practices get a different partner mix.
Setup takes two to three weeks. Most practices meet their first approved partners between weeks 8 and 12, and referrals become more consistent from months four to six. Some move faster, but that is the honest timeline.
Give us LinkedIn access so outreach runs in your name, show up to introduction calls, and approve or decline each partner within 48 hours.
No one can honestly promise how many patients a partner will send. What we commit to is written in your agreement: approved partners, replacement terms, and reporting.
It depends on your territory, specialties, and how many partners you need. We map that on the call. Programs are a one time fee, paid as setup plus monthly installments. We never take a percentage of your revenue.
Programs run on a 12 month plan so relationships have time to develop. Every term is in your agreement before you pay.
Our only website is practiscale.com, and every Trustpilot reviewer is searchable by name. If a website or number doesn’t match, contact us before you pay anyone.
We look at your specialties, panels, capacity, and territory, then tell you honestly whether we can help.
If we can, we show you the referral partners in your area. If we can’t, we tell you that too.
