Your Caregivers Are Ready. The People Who Refer Clients Don't Know You Exist.

Discharge planners, social workers, geriatric care managers, and senior living advisors recommend home care agencies every week. Most of them recommend the few agencies they already know. We get you introduced, warmed up, and onto that short list.

Home care agencies we have worked with

Private-duty agencies building hospital and discharge planner referral pipelines with PractiScale.

The problem

Your client problem is really a visibility problem.

Trained caregivers ready to work, waiting for hours while hospital referrals go elsewhere.

You got licensed. You hired caregivers. You wrote the care plans, set your rates, and got ready to say yes.

Then you waited.

One owner told us her caregivers call her every other day with the same question: when will you have hours for me? She had the staff. She did not have the clients to give them.

Other agencies have been open for years on word of mouth and repeat families. That works until it stalls, and the discharge planner at the hospital down the road still has no idea what your agency actually does.

So you tried what everyone tries. Ads that brought clicks and no calls. Flyers left at front desks. Networking events that filled an evening and nothing else. Lead sites that sold the same family to several agencies at once and stopped sending the moment you stopped paying.

Meanwhile, families who need you are being referred somewhere else. In the Home Care Association of America’s private duty benchmarking study, professional sources such as hospital discharge planners, home health agencies, skilled nursing facilities, hospices, geriatric care managers, and assisted living communities together produced about 44% of referrals. That is more than twice the share that came from past clients and their families.

For private duty home care, a hospital is not required to hand the patient a list of agencies. The discharge planner recommends the agencies they know and trust. If they don’t know you, you are not in the conversation.

Referral infrastructure is what we build. We build the relationships.

How PractiScale Builds Your Referral Network

Our team handles the outreach, the introductions, and the follow-up. Your job is to show up and deliver great service.

Step 01

We map the referral partners in your territory

We identify the discharge planners, case managers, skilled nursing social workers, geriatric care managers, hospice teams, senior living advisors, and physician offices that serve your zip codes and refer the kind of client you take. That means your care level, the hours you can staff, and the payers you accept. You receive a territory research report with every partner category mapped and prioritized.

Step 02

We reach out in your name and warm them up

Your dedicated coordinator runs outreach through your own LinkedIn profile, email, and phone, so partners get to know you, the owner. We explain your services, confirm they refer home care clients in your area, and screen out anyone without the volume or the fit.

Step 03

We book the introduction, and you decide

Qualified partners meet you on a short call, usually over Zoom. You decide whether they are the right fit. Nothing moves forward without your approval.

After the introduction: we stay involved so the relationship holds

One good meeting isn’t enough. After every introduction, our team manages ongoing follow-ups and touchpoints so your community stays top of mind the moment a family needs placement.

What we build for you in the first three weeks

Every system, script, and tracking workflow is built and configured for your agency before outreach begins.

What a referral source checks before they send you a family

These are the questions referral partners ask our team when we introduce a home care agency. Have clear answers and you become easy to refer to.

Often the first question

Who pays?

A director of social services asked us right away whether an agency accepted any Medicaid plans, because agencies that do are hard to find. A licensed social worker at a hospital asked whether the agency was self pay or insurance. Your payer mix decides which partners can refer to you at all.

Speed decides the call

How fast you can start care?

When a patient is cleared to go home, discharge planners often call several agencies at once to confirm availability and start dates. The agency that answers and confirms first usually gets that client, and often the next one too.

Where independent agencies win

Flexibility on hours

A retirement housing advisor told our team her client could not work with a franchise that required 12 hours a week. Partners remember the agency that says yes to the short shifts, odd hours, and short term cases others turn away.

Accountability builds trust

A name they can call

Referral sources want to know who answers when a care plan goes sideways. An owner who picks up the phone, keeps the same caregiver with the same client, and reports back after a referral is the agency they call again.

Don’t take our word for it. Here’s what the work looks like.

What changes when clients come from referral partners

Your caregivers get the hours you promised them

Caregivers without steady hours move on. Activated Insights put median caregiver turnover at 75% in 2024, and most caregivers who leave do so within their first 100 days. Steady referrals give you hours to offer the people you already hired.

Less dependence on word of mouth and lead sites

Past clients and their families will always matter. Direct partners add a second source that does not depend on who your last client happened to know, or on a lead you share with four other agencies.

Referrals without a cut

Past clients and their families will always matter. Direct partners add a second source that does not depend on who your last client happened to knAVow, or on a lead you share with four other agencies.

Referrals without a cut

A geriatric care manager or social worker who trusts you does not refer once and disappear. They come back every time a family on their caseload needs help at home.

The system stays with you

The CRM, the pipeline, and the relationships are built in your agency’s name. They stay yours.

Your time goes back to care

You are already scheduling caregivers, doing assessments, and answering families. Building a referral network on top of that is a full time job. Our team does it.

“Picture a Friday afternoon. A discharge planner calls because a patient is going home on Monday and needs a caregiver by the weekend. She calls you first, because she already knows you, your hours, and how fast you start.”

That is what a working referral network looks like. That is what we help you build.

We do not leave you with a partner who is not performing.

Every referral partner we connect you with is one you have approved. You meet them, you decide if they are the right fit for your coverage area and your vehicle types, and you say yes before we move forward.

Referral relationships are not always perfect from day one. If a partner we connect you with slows down, stops sending trips, or turns out not to be the right fit we go back to work. We find a replacement, warm them up, and make the introduction again. No extra cost. No long-term contract holding you in place.

We stay involved until you have referral partners who are actually sending trips consistently.

Replace-Partner Guarantee

If a partner stops performing, we find a replacement. No extra cost.

No Long-Term Contract

You stay because it is working, not because you are locked in.

You Approve Every Partner

You meet them. You decide. You approve.

Common questions from business owners.

A closer look at the process, the partners, and what to expect.

Lead sites sell you a family’s contact details, and the same family is often sent to several agencies at once. When you stop paying, the leads stop. We connect you with professionals who refer families again and again, and the relationship belongs to your agency.

No. Keep every source that works. Most agencies we work with started on past clients and families. Direct partners add steady volume that does not depend on your last client’s network.

Many agencies we work with start with a handful of caregivers and no referral relationships at all. What matters is that you are licensed, can staff a new client quickly, and can respond to a referral the same day. We will tell you honestly on the call whether you are ready.

Your setup is delivered in the first two to three weeks. Outreach and prequalification follow. Most agencies meet their first approved partners between weeks 8 and 12, and referrals become more consistent from months four to six. Some move faster. Nicole at Visiting Care Homecare had 15+ referral conversations in her first 30 days, and that was her result, not a promise. We would rather give you the real timeline now than apologize for it later.

Three things. Give us access to your LinkedIn so outreach can run in your name. Show up to the introduction calls. Approve or decline each partner within 48 hours, so we can follow up while the conversation is still fresh.

Nobody can honestly promise how many families a hospital will discharge next month. What we commit to is written in your agreement: the number of approved partners, the replacement terms, and the reporting you receive. Read it before you sign, and ask us about anything that is unclear.

Tell us. We review it, find a replacement, warm them up, and make the introduction again at no extra cost, within the replacement terms in your agreement.

Yes. We target partners who refer the clients you can actually serve. A private pay agency and a Medicaid waiver agency need different partners, and your territory research reflects that.

It depends on your territory, your services, and how many partners you need. We map that on the call and only recommend a plan that fits. Programs are a one time fee, paid as a setup payment with the balance spread over monthly installments. We never take a percentage of your revenue.

Look us up. Our only website is practiscale.com, and every reviewer on our Trustpilot page is searchable by name. Copycat sites have used names similar to ours, so if a website or phone number does not match practiscale.com, contact us before you pay anyone.
